Community health and referrals

We work with communities and public health systems to strengthen prevention, care, referrals and continuity of care.

Community context for this story

From the work

Community health promoters are often the bridge between general health information and a household’s next practical step. They listen, explain and help people recognise when home care is not enough.

The relationship works because it is local. Families can ask questions in familiar language and return to the same person if something is unclear.

Strong referral is more than telling someone to visit a facility. It means understanding likely barriers, explaining why care matters and maintaining a reliable link with health teams.
